Certificate of Insurance Request

To request a Certificate of Insurance (COI) for an AFHE event, fill out the form below. This form will automatically request a COI from our insurance provider via email. You and the locations coordinator will be included on the email and instructions to replay all will be provided to the insurance company so everyone will receive the COI once the insurance company produces it.
